The white smear is the easy half
Toothpaste is mostly water, a mild abrasive and a detergent — three things that already want to leave fabric. Nothing has bonded; it sits on the surface looking more dramatic than it is. The one reliable way to make this hard is to rub it, which grinds the abrasive into the weave.

This fits in the time it takes to find your keys. For anything that survives it, the University of Georgia Extension publishes a stronger soak — “1 quart warm water, 1/2 teaspoon liquid hand dishwashing detergent and 1 tablespoon of vinegar for 15 minutes” (remove stains from toothpaste).

The American Dental Association states that “whitening toothpastes primarily rely on abrasives for mechanical removal of extrinsic surface stains, though some contain low levels of peroxide to help lighten tooth color” (whitening), and calls hydrogen peroxide “a strong oxidizing agent.” An oxidiser cannot tell a stain on enamel from the dye in your shirt.
The levels are low and most people lose nothing. But colour loss is the one outcome no later treatment reverses, so the rule splits by garment: a white shirt waits until you get home, a navy one gets rinsed now (stains on colored clothes).

Does toothpaste stain clothes permanently?
Very rarely. Maytag notes that “even if the stain has been sitting, your washer’s normal cycle should remove it” (how to remove toothpaste stains). What is permanent is colour lifted from a dyed fabric — bleaching, not staining.
Can I use bleach on a toothpaste mark?
Bleach-safe whites only. Clorox warns on its own toothpaste page to “avoid bleaching wool, silk, mohair, leather and any item containing spandex” (toothpaste stains out of a shirt).
Hot or cold water for toothpaste?
Cool. Nothing in it needs heat, and heat fixes whatever you missed (does vinegar remove stains).
Why is there still a mark after it dried?
